- 加入 .gitignore：documents/index.json、runtime/MEMORY.md、USER.md、
  memories.json 是按机器/按人累积的本地私有状态
- 入库的代价：每次记忆写入都产生提交抖动，且可能带出本机绝对路径与
  个人偏好
- 上一笔只含 .mnemon/ 四个空骨架文件的提交（未推送）已撤销，历史里
  不含 .mnemon/；磁盘上的文件保持原样，只是不再被跟踪
