chore(skills): 纳入 template-skill 骨架（DSH 建 skill 的空白模板）

.dsh/skills/ 是入库的项目知识（.gitignore 用 `!.dsh/skills/` 显式放行），此前该目录未被跟踪，现按同一口径纳入。内容为 DSH 新建 skill 的空白骨架：description 仍是占位文案、正文未填。后续要落地具体 skill 时直接改写本文件；确认不需要则删目录即可。
